Each year, CBINT publishes a congregational Yizkor booklet which we use in the four Yizkor services we pray annually, beginning with Yom Kippur. The booklet includes listings from congregant households of the individuals they lovingly remember.We take orders for listings in the new year’s booklet each summer.

Yizkor Book of Remembrance Form – 2026

We are approaching the High Holidays 5787/2026-2027.  To remember beloved relatives and friends in the Yizkor Book of Remembrance, please complete and return this form with payment to the synagogue office by Friday, August 7, 2026.

Names to be included in your listing (first and last name) OR check box immediately below if same as last year.

If you have more names than will fit in the numbered section immediately below (10 names), use the text box farther down on the form to include the additional names.

You can pay for your Yizkor Book listing using PayPal or a credit card within this form.
There is a 3% fee added to all credit card transactions.

Or you can pay by a check made out to "CBINT" or "Congregation Beth Israel Ner Tamid" and mailed to:
6880 N. Green Bay Ave., Glendale, WI 53209.

You can also drop off a check during our office hours of 9:00am - 1:00pm, or leave it in the wall-mounted dropbox near the school/office entrance.
